A National Lipid Association (NLA) Nutrition and Lifestyle Taskforce reviewed the current scientific evidence to examine the effects of low-carbohydrate and very-low carbohydrate, including ketogenic, diets on weight loss, dyslipidemia, and other cardiometabolic risk factors, and developed a scientific statement to provide guidance for practicing clinicians on the use of these diets in clinical practice. Based on available scientific evidence, low-carbohydrate diets may offer short-term (<6 months) benefits, such as weight reduction in adults with overweight or obesity, and improved glycemic control in patients with pre-diabetes and type 2 diabetes. However, many of the short-term benefits are not maintained over longer periods of time. The limitations of low-carbohydrate diets, especially very-low-carbohydrate diets, are difficulty with adherence and lack of long-term data on cardiovascular outcomes and safety. Additionally, very-low-carbohydrate diets may severely restrict nutrient dense, carbohydrate-containing foods that are known to be associated with cardiometabolic benefits, including vegetables, fruits, whole grains, seeds and nuts, and legumes.
